What tests do I need before starting BHRT?

Before starting BHRT, the most important “test” is a careful clinical evaluation: your symptoms, menstrual or reproductive history, medical risks, medications, family history, and goals. Lab testing can be very helpful, but it should be targeted and interpreted in context rather than used as a one-size-fits-all checklist. Standard bloodwork is important, but it may not always show the full picture of how your body is producing, using, and metabolizing hormones. A risk-aware BHRT provider uses the right mix of history, standard labs, and specialty testing when needed to confirm patterns, rule out look-alike conditions, choose the safest route and dose, and create a monitoring plan.

Fatigue, weight gain, brain fog, poor sleep, low libido, mood changes, and hair loss can also come from thyroid dysfunction, insulin resistance, low ferritin, vitamin deficiencies, inflammation, stress physiology, sleep apnea, or medication effects. A good BHRT evaluation looks for these common drivers too.
  • Specialty testing should be actionable. DUTCH testing, saliva cortisol testing, hormone metabolite testing, or organic acid markers may be useful in select cases, especially when standard labs do not explain symptoms. These tests should be used when the results will help guide care, not as a routine requirement for every patient.
  • Monitoring continues after you start. BHRT is not a one-time lab decision. Follow-up should track symptoms, side effects, blood pressure, bleeding patterns, hormone response, testosterone-related markers when relevant, and updated labs when clinically appropriate.

Understanding BHRT Testing

BHRT stands for bioidentical hormone replacement therapy. It uses hormones that match the molecular structure of hormones naturally made by the body, such as estradiol, micronized progesterone, and testosterone. Testing before BHRT helps your provider decide whether hormones are likely to help, whether they are safe for you, and how to monitor treatment.

Testing does not mean chasing “perfect” numbers. Hormone levels change by age, cycle phase, menopause stage, time of day, sleep, stress, medications, and health conditions. This is especially true during perimenopause, when estrogen and progesterone can swing up and down before becoming consistently lower.

Standard lab ranges can also be limited. They often reflect what is statistically common in a broad population, not necessarily what is optimal for a specific person with specific symptoms and goals. A result may be “normal” but still not explain why someone feels off, especially if only a few markers were checked.

Comprehensive hormone testing looks at more than basic hormone levels. Depending on the case, it may evaluate hormone patterns, free versus bound hormone, hormone metabolites, thyroid conversion, cortisol rhythm, insulin resistance, inflammation, and nutrient status. The goal is to understand not only where hormone levels are, but also how the body is using and clearing them.

The best testing strategy is individualized. A provider should be able to explain why each test is being ordered, what question it answers, and how the result could change the treatment plan.

What Testing Should My BHRT Provider Run?

A responsible BHRT evaluation usually includes baseline labs that look at hormones, safety markers, and common conditions that can mimic hormone imbalance. The exact panel should be personalized based on your age, menstrual status, symptoms, medications, health history, and goals.

  • Sex hormone testing: For women, this may include estradiol, estrone, estriol, progesterone when timing is meaningful, total testosterone, free testosterone, sex hormone-binding globulin, FSH, LH, and DHEA-S. For men, this usually includes total testosterone, free or calculated free testosterone, sex hormone-binding globulin, estradiol, LH, and DHEA-S when appropriate.
  • Thyroid testing: A complete thyroid panel may include TSH, free T4, free T3, reverse T3, and thyroid antibodies such as TPO and thyroglobulin antibodies. This helps show whether thyroid hormone is being produced, converted into active T3, or affected by autoimmune thyroid disease.
  • Cortisol and adrenal rhythm testing: Cortisol follows a daily rhythm, ideally rising in the morning and gradually declining through the day. Saliva or dried urine testing may be used to assess cortisol patterns, the cortisol awakening response, DHEA-S, cortisol metabolites, and how stress physiology may be affecting energy, mood, sleep, and hormone balance.
  • Metabolic testing: Fasting glucose, fasting insulin, A1C, lipid panels, liver enzymes, kidney function, and inflammatory markers such as hs-CRP or homocysteine can help evaluate insulin resistance, cardiovascular risk, inflammation, and metabolic factors that may be driving symptoms.
  • Nutrient and anemia markers: CBC, ferritin, iron studies, vitamin D, B12, and folate may be useful when symptoms include fatigue, hair loss, brain fog, low mood, restless legs, poor recovery, weakness, or exercise intolerance.
  • Estrogen metabolism markers: In select cases, dried urine testing may help evaluate estrogen metabolites, including pathways such as 2-OH, 4-OH, and 16-OH estrogen metabolism. This can provide additional context for how the body processes estrogen, especially when symptoms, history, or risk factors make that information clinically useful.
  • Safety markers: Blood pressure, personal and family history, breast health, bleeding history, clotting history, cardiovascular history, and medication review are part of safety screening. For men considering testosterone, hematocrit and prostate-related evaluation may be appropriate based on age and risk.

Testing should also account for delivery route. A woman using transdermal estradiol, a woman using vaginal estrogen, a woman using oral progesterone, and a man using testosterone injections do not need the exact same monitoring plan. The route, dose, and formulation determine what should be watched over time.

Understanding Different Hormone Testing Methods

Different testing methods provide different types of information. Blood testing remains important and is often the best starting point for baseline health markers, thyroid function, fasting insulin, blood count, metabolic health, and many standard safety labs. It can measure hormones circulating in the bloodstream at the time of the draw, including total and free hormone levels when ordered appropriately.

The limitation is that blood testing captures a snapshot. For hormones that fluctuate throughout the day or throughout the cycle, one blood draw may not show the whole pattern. This is one reason symptoms and timing matter.

Saliva testing measures free, bioavailable hormones, meaning the fraction that is more readily available to enter cells and exert effects. It can be especially useful for assessing cortisol rhythm throughout the day. Timed saliva samples can capture the cortisol awakening response, which is the natural cortisol rise that should occur within about 30 to 45 minutes of waking.

Dried urine testing, often referred to as DUTCH testing, collects several urine samples over a day. This can provide information about hormone production and hormone metabolites, which are the breakdown products that show how the body is processing hormones. DUTCH testing may also provide insight into cortisol metabolism, melatonin production, oxidative stress markers, organic acid markers, B vitamin-related patterns, and neurotransmitter-related metabolism.

None of these methods is perfect for every situation. The question is not whether blood, saliva, or urine testing is “best.” The better question is which method answers the clinical question that matters for your case.

Why Timing Matters

Timing can change how hormone test results are interpreted. In cycling women, estradiol and progesterone fluctuate dramatically throughout the menstrual cycle. Progesterone is usually most meaningful when tested about 5 to 7 days after ovulation, often around days 19 to 21 in a typical 28-day cycle.

FSH and LH are often tested early in the cycle, commonly around day 3, when the goal is to evaluate ovarian signaling. In perimenopause, however, cycles can become irregular and hormone levels may fluctuate significantly, so results must be interpreted with more caution.

Men and postmenopausal women generally have more stable reproductive hormone patterns than cycling women, but timing still matters for some labs. Testosterone is often best checked in the morning, especially in men, because levels are typically highest earlier in the day.

Cortisol timing is especially important. A single cortisol value rarely tells the full story. Cortisol should normally follow a rhythm: higher in the morning to support waking and lower later in the day to support rest. Multiple timed samples can show whether cortisol is rising appropriately, crashing too early, staying elevated at night, or being metabolized in a way that affects energy and sleep.

Testing at the wrong time can lead to confusing or misleading conclusions. A good provider should explain when to test, how to prepare, and whether medications, supplements, cycle timing, or hormone use could affect results.

Why Symptoms And Labs Both Matter

One of the biggest mistakes in hormone care is treating symptoms without testing or treating lab numbers without listening to the patient. Both approaches can lead to poor results. Symptoms tell the provider what is happening in real life; labs help check patterns, rule out other causes, and create a safer plan.

For example, a woman with hot flashes, night sweats, poor sleep, and irregular cycles in her late 40s may be in perimenopause even if one estradiol result looks normal. Hormones fluctuate during this stage, so a single lab draw may not capture the full pattern.

On the other hand, a person with fatigue, weight gain, and low mood may not need BHRT at all. Those symptoms could come from hypothyroidism, low ferritin, B12 deficiency, depression, medication effects, insulin resistance, sleep apnea, overtraining, under-eating, or chronic stress. Testing helps avoid missing the real driver.

For men, low energy and low libido do not automatically mean testosterone replacement is needed. Proper testosterone testing, symptom review, medication review, sleep assessment, metabolic health evaluation, and safety screening should come first.

The best BHRT decisions happen when symptoms, labs, medical history, and risk profile all point in the same direction. Hormone levels that look “abnormal” may be appropriate for someone’s age, situation, or cycle timing, while levels inside a broad reference range may still be suboptimal for that person’s goals and symptoms.

Testing For Women Before BHRT

For women, the right testing depends on whether she is cycling regularly, in perimenopause, postmenopausal, using hormonal birth control, has had a hysterectomy, or has had the ovaries removed. These details change how labs are interpreted.

In cycling women, estradiol and progesterone vary by cycle phase. Progesterone is most meaningful after ovulation, not randomly at any point in the month. In perimenopause, ovulation may be inconsistent, so progesterone can be low some cycles and normal in others.

FSH and LH can help in certain situations, especially when early menopause, premature ovarian insufficiency, or unclear menopause status is being evaluated. But FSH can fluctuate during perimenopause and should not be treated as the only answer.

Estrogen metabolism may also be relevant for some women. The body breaks estrogen down through different pathways, and metabolite testing can sometimes provide additional context when symptoms, family history, estrogen exposure, or risk factors call for a deeper look.

If systemic estrogen is being considered, uterus status matters. Women with a uterus usually need progesterone or another appropriate progestogen to protect the uterine lining. Progesterone may also be appropriate for some women who no longer have a uterus, especially when it supports sleep, mood, or overall hormone balance, but the reason for prescribing it is different than uterine protection and should be individualized.

Women with heavy, irregular, persistent, or postmenopausal bleeding may need evaluation before hormones are started or adjusted. That may include pelvic exam, ultrasound, endometrial evaluation, or referral depending on the situation.

Testing For Men Before Testosterone Therapy

For men, testosterone therapy should start with proper diagnosis. That usually means symptoms consistent with low testosterone plus properly timed lab testing. Morning total testosterone is commonly used because testosterone levels are usually highest earlier in the day.

Free testosterone or calculated free testosterone may be useful when total testosterone does not match symptoms or when sex hormone-binding globulin is abnormal. LH and other pituitary-related labs may help determine whether the issue is primarily testicular or related to signaling from the brain.

Men also need estrogen in the right amount. Estradiol may be checked when symptoms, body composition, medications, testosterone therapy, or side effects suggest that estrogen balance could be contributing to the picture.

Safety testing matters before testosterone therapy. A CBC is used to check hematocrit, because testosterone can raise red blood cell production. If hematocrit becomes too high, blood may become more viscous, which is one reason monitoring is required.

Depending on age, history, and risk factors, prostate-related evaluation may also be appropriate before and during therapy. Recent research is more reassuring than older assumptions about prostate risk in appropriately selected men, but monitoring is still part of responsible care.

Sleep apnea, cardiovascular risk, fertility goals, medication use, alcohol intake, and metabolic health should also be reviewed. Testosterone therapy can reduce sperm production, so men who want future fertility need a different discussion before starting.

Who It’s For And Who Should Be Cautious

BHRT testing is appropriate for women with bothersome perimenopause or menopause symptoms, including hot flashes, night sweats, poor sleep, mood changes, vaginal dryness, painful sex, urinary symptoms, brain fog, low libido, or changes in body composition. It may also be useful for women with early menopause symptoms, surgical menopause, or unclear hormone status.

Testing is also appropriate for men with symptoms that may fit low testosterone, such as low libido, erectile changes, fatigue, depressed mood, loss of muscle, increased abdominal fat, poor recovery, or low motivation. Symptoms should be paired with properly interpreted labs before treatment is considered.

People who need extra caution include those with a history of hormone-sensitive cancer, unexplained vaginal bleeding, blood clots, stroke, significant liver disease, uncontrolled high blood pressure, active cardiovascular disease, untreated sleep apnea, very high hematocrit, or complex medication histories. These situations may require more testing, specialist input, or non-hormonal options.

Testing is also important for people whose symptoms could be caused by something else. If a provider skips thyroid, metabolic, anemia, medication, sleep, cortisol rhythm, or inflammation considerations, the hormone plan may miss the real problem.

The most useful testing strategy is targeted, not excessive. Comprehensive testing can be valuable, but only when it helps clarify the picture and guide a safer, more effective plan.

Risks, Side Effects, and Monitoring

Testing itself carries minimal risk. Blood draws may cause minor bruising, tenderness, or lightheadedness. If you have a bleeding disorder or take blood thinners, tell your provider before having blood drawn.

The bigger safety issue is interpretation. Self-treating based on hormone test results without professional guidance can be harmful. Hormone levels that appear “low” or “high” may be appropriate for your age, cycle timing, menopause stage, medication use, or current therapy.

Testing before BHRT helps reduce risk, but it does not eliminate it. BHRT can still cause side effects such as breast tenderness, bloating, headaches, nausea, spotting or irregular bleeding, skin irritation, mood changes, acne, fluid retention, sleep changes, or changes in libido. These are often dose- or route-related and may improve with adjustment.

Estrogen-related monitoring may include symptom response, bleeding patterns, blood pressure, breast health screening when appropriate, and reassessment of route and dose. Oral estrogen and transdermal estradiol behave differently, so route should be chosen intentionally rather than by habit.

Progesterone monitoring includes tolerance, sleep response, mood changes, grogginess, bloating, dizziness, and bleeding pattern when relevant. For women with a uterus using systemic estrogen, adequate uterine protection is a major safety issue.

Testosterone monitoring may include total and free testosterone, hematocrit, blood pressure, symptom response, acne or hair changes, mood changes, sleep apnea symptoms, and prostate-related considerations when appropriate. In women, testosterone should be dosed carefully to avoid levels that are too high.

Follow-up testing should not be automatic or random. It should be tied to the treatment used, the patient’s risk factors, symptom response, and whether results will change the plan. BHRT should be adjusted based on both how the patient feels and what safety markers show.

Safety

Seek urgent medical care right away for chest pain, shortness of breath, one-sided weakness or numbness, sudden severe headache, vision changes, fainting, calf pain or swelling, coughing up blood, or heavy vaginal bleeding. These symptoms can signal rare but serious problems such as a blood clot, stroke, pulmonary embolism, or significant bleeding.

Call your provider for non-urgent but important concerns such as unexpected spotting, heavy or prolonged bleeding, postmenopausal bleeding, persistent breast tenderness, worsening headaches, mood changes, acne, scalp hair shedding, swelling, or symptoms that worsen after starting therapy. Do not adjust your dose based on lab results without medical guidance.

If you take medications for blood pressure, blood thinning, thyroid disease, diabetes, seizures, mood, fertility, or cardiovascular conditions, hormone decisions should be coordinated with your prescribing clinician. If you are already under the care of an endocrinologist, cardiologist, oncologist, gynecologist, urologist, or another specialist, your BHRT provider should coordinate with that care team when needed.
