A standard comprehensive metabolic panel measures 14 substances — primarily electrolytes, glucose, and liver and kidney enzymes. A basic thyroid screen checks only TSH. This is where advanced testing diverges most dramatically from standard care. A conventional annual physical checks fasting glucose and maybe hemoglobin A1c. Advanced testing adds fasting insulin and HOMA-IR (a calculated ratio of insulin and glucose that estimates insulin resistance) — markers that can detect metabolic dysfunction years before glucose ever becomes abnormal. Your fasting glucose can look perfectly normal while your pancreas is producing two or three times the insulin it should be in order to maintain that number. Without measuring insulin directly, that early dysfunction is invisible.
Advanced metabolic panels also typically include a more detailed lipid analysis. Rather than relying solely on total cholesterol and LDL, a comprehensive panel may evaluate your triglyceride-to-HDL ratio (one of the strongest simple indicators of insulin resistance), LDL particle number and size, and apolipoprotein B — markers that tell a far more meaningful story about cardiovascular and metabolic risk than a basic lipid panel alone.
Complete Thyroid Panels
Standard thyroid screening checks TSH — and often stops there. TSH is a pituitary hormone that signals the thyroid; it is an important screening marker, but it does not tell you how much active thyroid hormone is actually reaching your cells. A complete thyroid panel includes TSH, free T4, free T3, reverse T3, and thyroid antibodies (TPO and thyroglobulin antibodies). Free T3 is the biologically active form of thyroid hormone — the one your cells actually use. Reverse T3 can block active thyroid hormone at the cellular level, especially under chronic stress. And thyroid antibodies can reveal autoimmune thyroid disease (Hashimoto’s) that may be driving symptoms long before TSH moves outside the standard reference range.
This distinction matters because many people — particularly women over 40 — have symptoms consistent with low thyroid function (fatigue, weight gain, cold intolerance, hair thinning, brain fog) but are told their thyroid is “fine” because their TSH is technically within range. A complete panel gives a far more accurate picture of what the thyroid is actually doing.
Hormone Profiles
Advanced hormone evaluation goes well beyond checking a single estrogen or testosterone level. A comprehensive hormone panel typically includes estradiol, progesterone, total and free testosterone, DHEA-S, sex hormone-binding globulin (SHBG), and cortisol. Breath testing can evaluate for small intestinal bacterial overgrowth (SIBO), a common and underdiagnosed contributor to bloating, gas, constipation, diarrhea, and nutrient malabsorption.

Food sensitivity testing measures delayed immune reactions (typically IgG or IgA antibodies) to specific foods.
