For women, important clues include cycle length, skipped periods, heavier or lighter bleeding, PMS changes, hot flashes, night sweats, vaginal dryness, painful sex, urinary symptoms, libido changes, mood shifts, and sleep disruption. Uterus status also matters because women with a uterus usually need progesterone or another appropriate progestogen if systemic estrogen is prescribed.
For men, important clues include libido, erectile changes, morning erections, energy, muscle strength, exercise recovery, mood, sleep quality, snoring, alcohol intake, abdominal weight gain, medications, fertility goals, and cardiovascular risk.
Risk review is just as important as symptom review. A provider should ask about hormone-sensitive cancer history, breast or prostate history, unexplained bleeding, blood clots, stroke, cardiovascular disease, liver disease, migraines with aura, sleep apnea, blood pressure, medications, and family history.

Practical gut support usually starts with basics: fiber, hydration, regular meals, protein, enough minerals, movement, and addressing constipation. More advanced gut testing or treatment may be considered when symptoms are persistent or clinically relevant.
When BHRT Or HRT May Help
BHRT stands for bioidentical hormone replacement therapy. Bioidentical hormones have the same molecular structure as hormones naturally made by the body, such as estradiol, micronized progesterone, and testosterone. HRT is the broader term for hormone replacement therapy and may include bioidentical or non-bioidentical options.
For women, BHRT or HRT may help with moderate to severe hot flashes, night sweats, poor sleep related to vasomotor symptoms, vaginal dryness, painful sex, urinary symptoms, and quality-of-life disruption related to perimenopause or menopause. Hormone therapy may also be considered as part of a broader bone-health plan in properly selected patients.
Route matters. Transdermal estradiol, such as a patch, gel, or cream, enters through the skin and largely bypasses first-pass liver metabolism. Oral estrogen passes through the liver first and may have different effects on clotting-related and metabolic pathways. BHRT or HRT may be appropriate when symptoms, health history, labs, and safety review suggest that hormone therapy is likely to help more than it could harm. It is often most favorable for many healthy women when started closer to menopause, but age alone is not the only factor.
People who need extra caution include those with a history of hormone-sensitive cancer, unexplained vaginal bleeding, blood clots, stroke, significant liver disease, uncontrolled high blood pressure, active cardiovascular disease, migraine with aura, untreated sleep apnea, very high hematocrit, or complex medication histories. These effects are often dose- or route-related and may improve with adjustment.
Estrogen-related risks depend on route, dose, timing, duration, and personal history. Oral estrogen passes through the liver first and can affect clotting-related pathways differently than transdermal estradiol. Transdermal estradiol may be preferred for some patients with clotting, cardiovascular, migraine, or metabolic considerations.
Progesterone monitoring includes sleep response, mood, grogginess, dizziness, bloating, breast tenderness, and bleeding pattern when relevant. If systemic estrogen is prescribed for a woman with a uterus, progesterone or another appropriate progestogen is usually needed to protect the uterine lining.
Progesterone may also be appropriate for some women who no longer have a uterus, especially when it supports sleep, mood, or overall hormone balance, but the reason for prescribing it is different than uterine protection and should be individualized.

Safety

These symptoms can signal rare but serious problems such as a blood clot, stroke, pulmonary embolism, or significant bleeding.
